Please read and complete the affirmation below. By checking this box, I acknowledge that I know that the completed Pastoral Recommendation Form must be submitted by Dec. 15 (preferred deadline) or Feb. 1 (secondary deadline). By checking this box, I affirm that I am a member in good standing of an LCMS congregation, I plan to major or minor in Theological Languages at CUW, and I plan to become an LCMS pastor. I have read 1 Timothy 3:1-7, and I do not know of anything that disqualifies me from the Office of the Holy Ministry. If I receive the Luther Scholar Guarantee, I will take part in at least four activities of the Pre-Seminary Student Association and at least one recruitment event per year. If I change my plans, my church membership, or my qualification for the Pastoral Ministry in the future, I will inform the Pre-Seminary Director of the change as soon as possible. I understand that I will no longer receive the Luther Scholar Guarantee if I make any of these changes.
